Medical Cannabis Price: Are Expensive Strains Really Better?

In pharmacies, there are often numerous cannabis strains to choose from – with surprisingly large price differences. While some flowers cost less than 5 euros per gram, others are 10, 12, or even more euros. This quickly gives the impression that the more expensive strain must automatically be of higher quality, stronger, or more medically suitable.
However, a look at the product range of our partner pharmacy CannaRoKo in Bitterfeld-Wolfen shows why this calculation doesn't add up: They offer numerous different cannabis products with a wide range of THC levels. The prices for flowers sometimes start at around 4 euros per gram. High active ingredient content is therefore no longer exclusively found in the upper price segment.
For patients, therefore, the price per gram alone is not decisive. The more sensible question is:
How much does a reliably effective and well-tolerated daily application cost me?
A strain for 8 euros per gram can ultimately be more economical than a strain for 4 euros if, after medical consultation, a significantly smaller amount is needed or the effect is more consistent. Conversely, a cheaper strain can also work just as well and significantly reduce monthly therapy costs.
A high gram price does not automatically mean a better medical effect.

Germany is supplied with medicinal cannabis both from domestic production and imports. Pharmacies can obtain corresponding products from various manufacturers and importers. This results in different purchase prices even for comparable THC levels.
Current availability also plays a major role. A strain may temporarily become more expensive if only small quantities are available or an importer cannot resupply. However, this does not automatically make it more effective.
The assortment of specialized pharmacies can often be divided into three price ranges: under 5 euros, between 5 and 10 euros, and over 10 euros per gram.

Between the cheapest and the most expensive example, with a monthly quantity of 30 grams, there is a difference of 240.30 Euros. Calculated annually, that would be 2,883.60 Euros.
This shows why the medical cannabis price for self-payers can play a significant role. Nevertheless, it would be wrong to always opt for the cheapest strain. A poorly suited strain can lead to more applications, more frequent changes, or a higher required quantity.
For those with statutory health insurance, the situation is particularly important. Cannabis medications can be covered by health insurance under certain conditions. These conditions notably include a severe illness, a lack of suitable alternative therapies, and the prospect of a noticeable positive impact on symptoms.
However, statutory requirements and reimbursement regulations can change. Patients with existing or planned cost coverage should therefore clarify the current status directly with their health insurance company and their treating practice. (As of: July 23, 2026)
Other cannabis medications, such as standardized extracts or medications with dronabinol or nabilon, are distinct from cannabis flowers. Which costs can be covered in individual cases must be medically reviewed and clarified with the responsible health insurance company.
Many patients initially compare strains based on their THC content. A flower with 30 or 34 percent THC appears stronger at first glance and therefore potentially more valuable than a strain with 18 percent.
For medical selection, this view is too simplistic. A high percentage initially only describes the active ingredient concentration of the flower. It does not indicate how much a patient needs, how well the strain is tolerated, or whether it is suitable for the particular therapeutic goal.
More THC does not automatically mean more therapeutic benefit. High-potency cannabis products can be associated with an increased risk of undesirable psychological and physical effects. Therefore, the general principle is often to aim for the lowest individually effective dosage.
Very high THC levels can, among other things, promote undesirable reactions such as restlessness, dizziness, concentration problems, or an unpleasantly intense effect. Furthermore, tolerance can develop with regular use. Then a larger quantity may be needed for a comparable effect.
Economically, therefore, it is not the highest THC value that is decisive, but the required daily amount.
Simplified example calculation:
Strain A costs 4 euros per gram and is used at 0.30 grams per day: 1.20 euros per day
Strain B costs 8 euros per gram and is used at 0.12 grams per day: 0.96 euros per day
Whether a smaller amount is actually sufficient is individual and must be assessed by a doctor. But the example shows: The more expensive strain per gram can theoretically lead to lower daily costs.
The more meaningful value is therefore:
Price per gram × actual daily dose required = therapy costs per day
Terpenes are aromatic plant compounds that define the smell and taste of a strain. They are often also attributed an influence on the overall effect. There are interesting scientific approaches for this so-called entourage effect, but clinical evidence is still limited.
Therefore, it cannot be reliably deduced from a single terpene whether a strain is calming, activating, or suitable for a specific medical condition.
Nevertheless, the terpene profile can be a useful comparison feature – especially if a patient has already documented which strains they tolerate well. However, it should not be understood as a fixed promise of effect.
Perhaps the most important difference to the unregulated market of the past is that medical cannabis flowers from pharmacies are subject to binding pharmaceutical regulations.
For cannabis flowers, requirements include active ingredient content, potential contaminants, foreign matter, and pharmaceutical quality. These basic requirements apply not only to expensive products.
A cheap pharmacy flower is therefore not comparable to an uncontrolled product of unknown origin. The lower price does not automatically mean that laboratory tests or pharmaceutical standards have been omitted.
However, differences can still exist, for example, in:
Residual moisture and flower texture
Size and structure of individual flowers
Intensity of smell and taste
Packaging and storage
Availability of consecutive batches
Fluctuations within permissible specifications
Personal perception and tolerance
These characteristics can be relevant for patients. However, they do not automatically justify every price premium.
International studies on deviations between declared and measured THC values must also be correctly classified. Results from foreign recreational markets cannot be easily transferred to German pharmacy products.
For German patients, therefore, batch-specific analyses, proper storage, and personal therapy documentation are more informative than general experience reports from other markets.
A common mistake is selecting based on only a single characteristic:

None of these characteristics alone is sufficient to assess medical suitability.
Particularly important are the therapeutic goal, the prescribed dosage, possible concomitant diseases, other medications, previous experience with THC, and the type of application. Even a product that is positively rated by many other patients may be individually unsuitable.
The medical cannabis price can be a selection criterion – but not the only one. A comparison based on several practical factors is more informative.

If possible, do not change several factors at once. If the strain, quantity, application temperature, and time of day are changed simultaneously, it will be difficult to determine later what caused the difference.
The cost per treatment day can be easily calculated:
Price per gram × daily quantity in grams = daily product costs
For example, for a strain costing 7.99 euros and a medically determined daily quantity of 0.20 grams, this would be about 1.60 euros per day, or about 47.94 euros in 30 days.
Additionally, the following should be checked:
Is the strain regularly available?
Are shipping costs incurred?
Is there a comparable alternative with a similar THC/CBD profile?
Will the required amount remain stable over several weeks?
Does the strain actually improve the defined therapeutic goal?
Are the costs and perceived benefits in a reasonable relationship?
Another factor is tolerance development. Those who regularly use high amounts of THC may experience a reduced effect with the same amount over time. An independent dose increase is not automatically the right solution. Changes should be discussed with the treating physician.
Expensive cannabis strains are not necessarily better. The price can reflect higher production costs, smaller batches, tight availability, or a specific brand positioning. However, it is not a reliable assessment of therapeutic suitability.
Even a high THC content does not automatically justify a higher price. Very potent flowers can already be available in the medium or low-price segment. At the same time, more THC can increase the risk of unwanted effects and rapid tolerance development.

The most sensible comparison is not "Which strain is the most expensive?" but:
Which strain reliably achieves the jointly defined therapeutic goal, is well-tolerated, and at reasonable overall costs?
No. Highly potent flowers are now also available in the low to medium price segment. The price is influenced by the manufacturer, importer, availability, production volume, pharmacy calculation, and THC content.
The differences arise, among other things, from the country of production, purchasing conditions, batch size, import costs, availability, processing, and the pharmacy's effort. A scarce or well-known strain can be more expensive without being medically superior.
THC is an important component of the active ingredient profile, but not the only selection criterion. Equally relevant are CBD content, required dose, personal tolerability, concomitant medication, therapeutic goal, and previous experience. The importance of terpenes for medical effects is still being researched.
That is possible. Even inexpensive medical cannabis flowers must meet the applicable pharmaceutical quality requirements. Whether a strain is suitable depends, however, on the individual effect and tolerability. A low price no more guarantees a good fit than a high price.
Helpful are a medically supervised selection, consistent use, and structured documentation of effects, side effects, daily amount, and costs. Instead of just comparing the price per gram, the actual therapy costs per day should be considered.
